- The Sacramental preparation program fee is $100. This includes materials, retreat expenses, and snacks/meals. Please submit your check (payable to Good Shepherd & include Confirmation & Teen name in the memo line) (DUE April 30, 2023).
- Confirmation Agreement and Contact Information Form (DUE April 30, 2023).
- Copy of Baptismal Certificate (DUE April 30, 2023).
- Diocese of Pensacola-Tallahassee Parent/Guardian Permission for Direct Contact (DUE June 1, 2023).
- Saint Name Information Online Form (DUE November 14, 2023).
- Sponsor Certificate Form (DUE November 14, 2023).
